Care Quality Commission warns private firms threaten England's National Care Service

Private companies buying up care homes for older people in England could undermine the governmentโ€™s plan for a universal care service, the Care Quality Commission (CQC) warned in its annual report released this week. The regulator said the rapid consolidation of the sector, driven largely by US property investors, threatens the quality and affordability of care for the nationโ€™s most vulnerable.

The CQC data show that the five biggest firms now own one in seven care homes, a 26โ€ฏpercent rise in market share over the past two years. Smaller homes, especially those that combine residential care with nursing, have fallen by a quarter in the last 14โ€ฏyears. At the same time, homeโ€‘help services are fragmenting into tiny operations, with almost a third of providers caring for four people or fewer. The regulator said these trends are โ€œlargely by accident rather than designโ€, reflecting economic pressure and councilsโ€™ struggles to purchase services for people who rely on state funding.

The report flags several risks. Consolidation could push prices higher and reduce the personal touch that smaller homes traditionally provide. The funding system remains fragile, with most users either paying themselves or receiving limited state assistance. In some regions, providers are turning increasingly to selfโ€‘funders, making it harder for local authorities to secure care for those on public support. CQC chief inspector Chris Badger said the market reshaping needs to be guided by peopleโ€™s needs, not profit motives. Health Foundation analyst Hugh Alderwick called the findings โ€œmassive challengesโ€ and urged immediate action to give councils the money, skills and leadership they need. Care Englandโ€™s Martin Green warned that without clear policy support, smaller providers may become unviable as employer national insurance contributions, inflation and rising energy costs squeeze their margins.

The regulator says the governmentโ€™s renewed focus on social care, a priority for Prime Minister Andyโ€ฏBurnham, must be matched by concrete steps to protect the sectorโ€™s fundamentals. It calls for tighter monitoring of market changes, clearer planning for homeโ€‘help services and targeted measures to keep small homes alive. If the state can reshape the market around quality and affordability, it could safeguard the universal care service the government promises; if not, the sector may drift further into profitโ€‘driven consolidation, leaving older people with fewer choices and higher costs.
